The Concept Plastic surgery is the practice of reconstructing body parts. Rhinoplasty (nose repair) involves taking a skin flap from the forehead or cheek and using it to heal a broken nose. The blood supply must remain intact as the new skin heals.

The Story In 1793, two British medical officers watched a humble Indian potter perform a “miracle”: he reconstructed the nose of a soldier who had been mutilated in battle. This wasn’t a new trick; it was the Sushruta method, documented over 6,500 years ago. Sushruta realized that a flap of skin could be taken from the forehead and rotated onto the nose—but it had to remain attached by a “pedicle” to keep the blood supply flowing while it healed. This “Indian Flap” remains the gold standard in modern reconstructive surgery today.

Sanskrit Shloka: नासिकाप्रमाणं पृथिवीरुहपत्रम्… गण्डपाश्वार्त् । त्वक्ं उत्कृत्य… नासिकाग्रं रूधिरबद्धं सीव्येत् ॥

Transliteration: Nāsikāpramāṇaṃ pṛthivīruhapatram… gaṇḍapāśvārt | Tvakaṃ utkṛtya… nāsikāgraṃ rūdhirabaddhaṃ sīvyet || Sushruta Samhita (Sutra Sthana 16) (Describes the leaf template and flap method) .

Meaning: “Measuring the size of the nose with a leaf (Patra)… cut a flap of skin from the cheek… attach it to the tip of the nose… and suture it quickly while the blood is still flowing.”

 

Related Innovations The Sushruta Samhita detailed Karna Sandhana, which contained 15 distinct ways of otoplasty (ear repair). It also developed the concept of the ‘pedicle flap’, which connected skin grafts to a blood supply after transplantation.

Fun Fact British medics were astounded when they witnessed this procedure in Pune in 1793, carried out by a potter. They brought the method back to London, launching the modern field of plastic surgery.
